Implementation of a Pharmacist-Led Social Determinants of Health Screening Intervention during Transitions of Care in an Acute Care Setting
Description
The University at Buffalo (UB) School of Pharmacy and Pharmaceutical Sciences is conducting a social needs (or basic needs for humans to survive) survey study with patients of Buffalo General Medical Center (BGMC). The goal of the study is to help identify community-based organizations that may help patients with their social needs after discharge.

Participants will be enrolled in the study for about 3 months. They will complete a social needs survey with a trained research staff member while patients of BGMC. For each identified social needs, they will be given a list of community organizations to follow-up with after discharge.

Adults 18 years or older Patients admitted in Buffalo General Medical Center Fluent in English
